PDR gave some details about what might be causing the increasing failure in an interview with iTWire. They indicated possible cause for failures cold be improper storage and transportation, improper removal and simple mishandling. I think the increase in failure could also just be because of more widespread adoption of USB drives. The more you have in circulation, the more there are to fail.
